STEPS TO THE ADMISSION PROCESS

First, please contact us to make an initial determination that your child may be a good fit for our program.  Call us or fill in the online inquiry form to begin the process.  Once we have had an initial contact, these are the steps you will take for admission.

Step 1. Complete the application (see link below) and submit it by mail: 4025 County Road 178, Jay, FL 32565 or by e-mail to: intake@myfathersarrows.org 

Step 2. Begin gathering necessary admission documentation: medical records and recent physical, immunization records, dental records, school records, counseling records, foster care records if applicable, adoption documents if applicable, copies of insurance cards, custody documents if applicable, original birth certificate, original social security card, and guardians driver’s licenses. 

Step 3. After we receive your application and do an initial review, we will contact you with any further questions and give you the opportunity to ask any additional questions about our program. In the meantime, please look over our list of frequently asked questions. 

Step 4. Discuss any financial questions with our controller. 5. When we receive all documentation, we will call you to schedule your on-site interview with you and your child. 6. After the on-site interview, the admissions committee will meet to make a final decision on admission and notify you by 5pm of that day. If your child is not accepted for admission, no explanation will be given. If you decide not to proceed with admission for your child, no explanation will be expected. We want all parties to be totally peaceful in making the best decision for your child. 

Step 5. When we receive all documentation, we will call you to schedule your on-site interview with you and your child.  

Step 6. After the on-site interview, the admissions committee will meet to make a final decision on admission and notify you by 5pm of that day. If your child is not accepted for admission, no explanation will be given. If you decide not to proceed with admission for your child, no explanation will be expected. We want all parties to be totally peaceful in making the best decision for your child.  

Step 7. If accepted for admission, your child will be admitted at 10 am the following day.
